How much medical risk is present today?

Anticipated or active withdrawal, current intoxication, pregnancy, and any concurrent physical-health concern that could complicate ambulatory care.

Non-specialists often miss

Answer the prompts one at a time. A suggested starting point, confirmation items, and draft note language will appear when there is enough information to use.
Educational reference for clinicians, not a clinical assessment, placement determination, or medical or legal advice. Final placement decisions are made by a licensed clinician with full access to the patient and to your facility's required medical-necessity framework. Provider-neutral by design — no specific program is recommended. Runs entirely in your browser; no PHI is collected and nothing is saved or sent.
